[757labs] VOIP providers?

Can anyone on the list recommend a robust VOIP/SIP business class provider?

At the small office I work for, we dumped Cox last year and invested
in  Cisco hardware and Simple Signal but the service and tech  support
is lacking. They are close to bringing Cox back in just to get back
some trivial in-office features that are missing.

I can setup an Asterisk or Trixbox but in the end, the call has to
terminate somewhere.
